To request a transcript you must login or register by creating a new account. If you have already registered, please enter your Email and Password to the left and click Sign In.Please keep in mind that this login is separate from your my.CSMD online services login.

If this is your first time using the online order system, please click Create Account and enter the required information. Once your account is created you will be able to request transcripts online and track the status of your orders.

***NOTE: Please use either Mozilla Firefox or Google Chrome as your internet source.***

  • An Official Secure PDF transcript, sent by email to the recipient of your choice $5 per copy
  • An Official Paper Transcript, sent by regular mail $5 per copy (plus postage) or pick-up in person $5 per copy
***Attachments are sent "as is" when you attach them. If you have an form that needs the Registrar's Office to complete information, please e-mail it to before you make the transcript request. The Registrar's Office will complete and e-mail it back to you so that you may then attach it to your electronic request. Attachments can only be sent electronically. If you believe you cannot send it electronically, please contact the Registrar's Office to discuss any options.

***Current students please note: Recent grades, degrees or other information may not be on your record. Before starting an order, log into my.CSMD and check your unofficial transcript, making sure that everything has posted. Orders cannot be held for grades or degrees.***
